Weird, both my w202s have never shown any sign of headliner sagging. Could it be something you find on vehicles which live their lives outside in the sun, snow or rain?

Environmental exposure may accelerate the process but not prevent it unless you can stop time. The headliners are "padded" with a layer of foam about 4mm thick. The foam deteriorates with time. Eventually the foam can no longer support the fabric of the headliner and it sags. I had a 1995 C280 until recently and it never sagged - the car was outside for most of it's life. All our other W202's and my garage queen W210 failed. Even the foam seals in my Olympus OM-1D failed after 20 years. The open cell foams used in this application will fail at some point. Consider yourself fortunate that you haven't dealt with this yet.
