Most likely a bad solder joint on the circuit board on which the headrest motor sits.
This is from my '99:
-Remove two screws on lower seat back, remove back panel from seat.
-Check the headrest assembly with a bright light so you can see what pieces need to be moved aside then release the plastic bracket (it is tight but do-able without breaking anything).
-Tilt seat back so you can remove the headrest.
-Remove headrest.
-Remove motor and circuit board assembly. You'll have to figure out how to pop off the plastic cover but again, it is possible. Bright light and small screwdrivers.
-Slip out the motor and worm gear assembly. Check for cold solder joints (dull ***** of solder, not shiny and sleek). Touch up with a solder iron and some fresh solder.
-Reassembly is the reverse of assembly.
