Dunno how you can answer that without a lot more information, specifically what the mileage is and how it has been cared for. If it's been parked outside, it will age like a southern california beach bunny: Soon it will look worse than Wendy from Breaking Bad, and no feasible amount of restoration will return it to youthful beauty. And as noted, not all extended warranties are alike.
Here's the key: You can find low mileage, garaged MB's for cheap. Why? People fixate on maintaining them at the altar of Deutsche marks, the MB dealership. Most are faithful to the service instructions. This becomes a costly habit out of warranty, and after the first four-digit repair, they decide to sell it. With a good independent mechanic, and intestinal fortitude, you can get a used one, low mileage, well maintained, for cheap.
The extended warranty may help, but only if you find one that needs repairs the warranty company wouldn't know about. It's not a warranty, it's an insurance policy. If you can afford the risk, go without. If you anticipate expensive repairs, and are confident you can get payment for them from the company, go for it.
