The bottom line is that the compact spare that comes with the W221 models, fits perfectly on the non-AMG W222's. The factory W222 jack & tool kit is usually available on eBay. I carry both in the trunk of my 2015 S550 and will be transferring them to my 2018 that is on order. It takes up a bit of trunk space, but gives great peace of mind while driving outside large metro areas.

any concerns using a 18" spare on a car with 19" wheels and 4matic? wont the diff wheel diamter cause issues with the drivetrain?

I think that no matter what the size of the rim of the wheel sizes the same.
Also, the other day I had a flat tire. Mercedes came and put my spare on and I went to a tire shop and bought a new tire. I really like the non-runflat tires better than the runflat's. I saw the link above for a compact spare and even though it's in the UK I might try to buy one

My car originally came with the plus one package from the factory, 20". The spare was 19" from the factory. It's also 4matic. No issue. I'm sure the manual states to use the spare just as temporary for no more than 100 miles like ever other bicycle spare.

Here you go. 2015 S550 with 2013 W221 spare. They could have left room for it under the trunk floor, but they didn’t want us to have it.

Unlike the E Class cars, when one orders the AMG Sports Package, or what is now known as simply the AMG Line, on the W222, the brakes are the same as without the AMG package. You have to go to an S63 or S65 to get the big brakes.

Now I know how Streamliner feels about not having a spare in the trunk. I hit a large bolt in the road today while driving my Honda Accord blowing out the left rear tire. Pulled over , removed my work equipment out of the trunk , lifted the floor of the trunk and NO SPARE TIRE. Had to call Honda Hazard number to get picked up by a tow truck, drove me to the Honda dealership and $245 and 2 hours later I'm back on the road. The people that handled the situation did a fantastic job but we all talked about how all cars are doing away with spare tires and either giving you a tire pump or a can of fix0flat. First time I've ever been stranded on the road.
But the Honda doesn't even have the space for a donut tire much less a real wheel/tire combo for a spare.
Looks like though I will get a spare of some type for the Mercedes as this was not a pleasant experience.

I’m shocked that Honda doesn’t have a spare in their wonderful Accord. I’ll bet you just love opening the huge trunk of your Genesis G90 (photos below), gazing at all that open space and then lifting the beautifully carpeted floor to reveal that peace-of-mind-inspiring spare, jack & tools. MB and other makers truly have their collective heads up their *** on this issue, for seemingly no good reason. What a shame.
