The balance shaft in the early Mercedes-Benz M272 V6 engines was known to have problems due to soft metal being used in the manufacturing process of the timing gear. Over time, this gear wears down, causing timing to be thrown off.

Since you have a 2010 model year car, it is unlikely that you will experience the failure of the balance shaft sprocket due to soft metal. This issue was corrected by then.
However, the most likely first symptom you would see of this issue would be a check engine light with a P0016 or P0017 code indicating Crankshaft / Camshaft Position Correlation Error.
