Engine making noise w204

Can anyone please help me identify what might be causing this horrible noise. I don't know if a little history might help or not, but I bought this car about 1 year ago and have had a lot of problems with it. Right after I bought the car I noticed the cigarette lighter was not working and since it was still under warranty I decided to just get it fixed at the mercedes dealership. I also had them change the oil while it was there. They told me the car needed to be kept because it needed a new rear main seal / tail light wiring / struts and a axel. After a few days they called me and told me that the part of the warranty had been denied by mercedes because the original owner never had the oil change......I bought the car with 40k and this was around 45k. They did end up covering the rear main seal but saiid they would not cover anything else after this. About 3 weeks after that the alternator went out and was replaced.

Yesterday we took about a 400 mile trip and everything was fine. Today when I started the car there was a horrible sound coming from the engine. There was also a puff of white some coming from the exhaust. This car has never smoked or made any engine noises before.

I am honestly very worried and hope this isn't anything major. The car just turned over 50k

Any help would be appreciated........thank you !

The previous owner never changed the oil in 40K miles? Or maybe they changed it themselves and didn't reset the service the service indicator. Mobil 1 is good, but 40K and never a change? I can't imagine the condition of that oil if true. Struts at 45K miles? Seems extreme. The tail light issue is a recall. They changed the alternator - did they reset the serpentine belt correctly, you will have to tell us what it sounds like.

I honestly don't know if the first owner changed it or not but I wouldn't be surprised.....after buying the car and really going over it I do not believe it was really taken very good care of. Just my opinion !

I did the alternator. The belt does squeak a little at start up now that the alternator has been replaced.
