Continental 17" runflats and gravel roads
I am based in South Africa where the smoothness of the road surfaces does not compare to those in Europe.
The set of run flats drives me crazy and above 120kpu one can hardly have a conversation in the car, a 2015 C180 Exclusive.
These tyres have done approx 25K and honestly i cannot endure the road noise anymore.
What alternative brands can you recommend which will have the least road noise?

You have about 15K miles on your original tires - if you have Sport Version (wider rear tires) you have not be able to rotate front/rear.
Frankly - depending on you wheel-size, tire-pressure, and drive style you could be at the last 25%-30% of tread life... and YES going into the last 25%/30% especially on Extended Mobility tire things will be noisier, harsher, and less enjoyable.
YES - you will probably benefit a lot from moving forward to fresh tires - I choose "regular" tires as I have not had flat tire issues and carry a can of flat-fix.
I have 18K miles - 29K km - and have over 50% tread left on a decent set of "regular" tires.
Cannot give you recommendation since tire brands and tire style names in a particular brand etc are all entirely different between US and SA - here in US there is a UTQG "treadwear rating" where higher the # the longer that tire is expected to last (at proper MB recommended tire pressure) - the lower the # the intent is that will be a "stickier tire" (softer rubber) and less treadlife.

If you are actually driving on “gravel” roads, I would expect lots of road noise no matter what tires are used.
