Almost $125.00 per spark plug? $428.17 for oil and filter? My corvette takes 10 quarts of Mobil 1 (dry sump) and costs much less than that. Is this quote in US dollars? Do they use gold single use disposable tools? At the very least I would get another estimate from an independent garage.
Estimate looks right for MB standard A service with spark plug adder. Surprised they are not doing brake fluid flush and calling it a B service.

4. What Fuel Additive should I buy to add myself? Is it as easy as pouring the right amount into the tank? i like the liquimoly dijectron but anything with high amounts of PEA would be a good idea. 5. Anything else to note? only DIY if you know what you are doing, otherwise take it to an indy shop
Agreed with @MatthewJ There's no black magic involved with any of these services. I just did NGK spark plugs from FCPeuro. You will need a 14mm wobble slim wall spark plug socket and a torque wrench, but it's otherwise a VERY easy service. I got a 14/16mm set on Amazon for $16. If I still have the car long enough, would be a 1 beer(hour) job, tops. Again, FCPeuro or elsewhere for the air filters at half the price of what they're charging on this quote.

Definitely on the high side for all those quoted but I don't know what is your local dealership's door rate (hourly labour rate). Agree with everyone else who contributed to your thread, definitely DIY or let an independent shop do it, our cars are super easy to work on.
