Took a while but Carbonio called me directly through the help of Vividracing. Spoke to head of technical development and went through all possible scenarios. Will run another test shortly but we suspect it maybe a bad maf sensor on drivers side. Again can't argue I was trouble free for over a year so will do this test. It's the only thing which wasn't tested. If thing don't work out, I get a new box....hopefully.

If one MAF is faulty why isn't the problem showing with the OEM boxes?

Eventually will swap. First suggestion was to clean old maf sensors with cleaner, reinstall and test in carbonio. If cel occurs, use row box maf sensor in carbonio to test. If cel again then it's the carbonio box. If no cel, then old maf from original air box is faulty even after being cleaned. I am planning step one now and have already removed, cleaned and resealed old Maf sensors to carbonio. I thought my mechanic cleaned the Mafs when he cleaned boxes the last time I went thru the cels. He said he never cleaned them so after 39k miles this is the time I cleaned them. I am thinking one maf maybe not be working well with carbonio or is way dirty.

Update:
Ok Carbonio was disassembled and cleanned off the silicone used in the last test. Removed MAF sensors and cleanned it 2x with MAF sensor cleanner. reinstalled and sealed with fresh silicone.
Ive completed about 400 miles so far and no cel light with the carbonios. Normally i would last between 75-125 miles before it kicked in. So far so good and I think this may have done the trick.
Thank you for Rob at Carbonio who walked me through the process and help pinpoint some weak areas.

I am thinking it was a bad driver side maf and not seal cause it worked for over a year without issue. Giving it to 750-1k miles to see before I open my mouth and say it's fixed. If a bad seal then cel would have appeared almost immediately. Carbonio told me some Mafs have lower tolerance zones when running a box pushing more air over it especially at low idle. They see the issue often in audi Mafs often. The box does not warp or deteriorate over time despite discoloration. If it happens again then swap Mafs and check. If occurs with new Mafs then I guess I get a new box under warranty.
