What are the mains concerns?

Depends largely on the service history. At 120k it should have new:
spark plugs (suggested to be changed at 60k)
motor mounts (they may need to be changed again depending on when they were replaced the 1st time)
injectors (usually replaced around 70k)
Pulleys (usually replaced around 70k)
Trans/rear diff service (40k interval)
PCV valve (replacement time varies)
Oil changes - should be done ~5k miles
Camshaft adjusters - The dreaded rattle on cold or sometimes warm starts that goes for a few seconds. Common issue. Several ways to deal with it.
Wear on camshaft lobes/lifters.
The wear on all items will largely depend on how it was driven. At 120k, the suspension parts may need to be replaced as well. Also, things that could be issues on an MB with said mileage: rear main seal, various gasket leaks, bad sensors, power steering pump, etc.
Do a PPI at a dealer to get an idea of what you're working with.
