Car hops out of gear?

Car hops out of gear?
Anyone have the same probem and maybe a solution. Usually when i first start the car ill be driving and after i stop and start again, it seems as if the car puts itself in nuetral. Also sometimes hops out when i slow down and then speed up again. Only happens once in a while but is very anoying when it does!
