I've seen trannies start to give as low as 45k... mileage really isn't the issue, its age in time and level of abuse that matter most. The time to do a tranny flush (ironically enough) is when you car feels perfectly fine and its doing well... once you start noticing problems.... at that point its starting to get too late (which is why nobody does tranny flush at the right time, most wait till its way too late).

A new rebuilt is probably $6k, but you don't need that, just have yours rebuilt for a 1/3rd the price.
