Coolant and titanium in oil

OK MB gurus, from my oil sample I have some glycol in the oil. I haven't got the results from the new oil, but they will be coming soon.
What is the most comon source? It is 0.01 percent, and on a scale from reportable to severe it ranked as reportable.

I have the gasket for the oil cooler, guess I am doing that in the future.

How common are the head gaskets to go? There is not a drop of oil or coolant anywhere on the engine.
