Belt tensioner Question... I know another one.

So my brother had a stripped belt tensioner adjuster bolt/stud and when we were tighting up the belt, and just when the indicator dial on the tensioner started to move we heard a snap, and his new tensioner adjuster had snaped, almost like a bad casting where the pin goes through the "sleeve". Do tensioners fail and get harder to move? I took it out again and put it in the vice and tried to give it some torsional load and it was stiff( i got the dial to move about half way with my arms), but I don't have a new one to compare it against.
Doing the work ourselves on his headgasket has tested his nerves as little things here and there needed to be replaced and took time to have shipped.
I am at a loss for words as I have installed or worked around these types of benz tensioners before and never had issues with them.
Hope you might have some experiences or thoughts fellow Benzophiles.
