I've always loved this style but don't know much.

Rare 5sp and 4cy or was it swapped?

Input is appreciated

I could be wrong but W124s were a part of my life for over 11 yr and I do not recall that the W124s were ever offered in NA with anything smaller than the 2.8 L Inline 6. They were originally the SOHC 3L then the DOHC 3L and then the DOHC 3.2L and I do not recall them coming with anything but an auto transmission.
The car may have originally come from Europe with the smaller engine and manual box.
It could also be something from a 190E but I doubt that.
In my opinion the car is European or a swap was made.
Plutoe probably has the best solution. Get a Data Card.

That's an M111 (2.3 DOHC) and was never offered in the w124 platform. The description sort of says it all "Custom 2.3l turbo engine - 5 speed manual transmission (new clutch)". I hope this helps.

This engine M111.974 (and M111.973) was used in the R170 first generation SLK230 with super charger with a manual box. That car is readily available in NA.
It was never used in the W124s in NA.
What a waste of a prefectly good car. The W124 Cabs were and are beautiful to me. He had the M104 DOHC 3.2 L IL6 to work with that was, as I recall 220 HP.
This set up never would have come close and manually shifting the 4 spd my bet is the M104 would have cleaned the clock of the M111 supercharged.
