Exhaust problems (?) on 98 E 320

Exhaust problems (?) on 98 E 320
My friend's father has a 1998 E 320 with about 77,000 miles.
Lately, there has been a loud noise coming from the exhaust/muffler area while driving. Everytime you accelerate, one can hear a loud rumble inside the vehicle.
Do you think there is a hole in the muffler or the exhaust? I would greatly appreciate any opinions or comments. I imagine someone may have had this problem before.
