You can find the trans fluid fill tube near the firewall, slightly towards the passenger side of the car. It will have a black cap with either a red or black square plug in it. If the plug is black, the fluid is original. If the plug is red, it's been replaced.

If you do change it, you'll want to drain it, change the filter, and refill with new fluid. I'm also not sure this will fix all your problems, but it should help with the rough shift (not sure about the going into neutral).

I would start with changing the fluid and filter, and see if that solves your problems, but with that type of mileage on original oil (or just that type of mileage period) you may have other problems.
