I lose the microphone on my setup on occasion as well. To get it back, I remove the puck as a paired device on the phone (don't physically disconnect the puck, unpair and remove it (delete its profile) in the phone menu), and then I take the battery out of the phone, wait a few minutes, reassemble and re-pair the phone with the puck. It's a lot of trouble, but at least it works for me-until I lose it again.
I couldn't tell if you were actually removing the puck as a paired device from your phone.
The puck works well on some phones, not so well on others (like my very low tech Samsung).
There may be a software update for your phone which will improve things. It is worth checking for.

I have exactly the same problem with ViseeO MB-4 BT adaptor.
Phones pair ok, I can hear everything ok, but they can't hear me or can hear but very very faintly.
It's possible that those orange fiber optics don't need to be attached. I have a E 320CDI -07 with two orange wires going from comand module (in left side trunk) to a box (in front of spare tyre / behind back seats).
-Removing the fuses didn't work
-unplugging the optics didn't work
-upgrading firmware in BT-4 didn't work.
... don't know much to do, except that in one discussion I found out that with some key kombination you should be able to open a mic volume adjustment, that makes it possible to re-adjust the factory presetting. Haven't still found the right combination.
