Out Of Control!!
When the tensioner is faulty-----have you tried to decrease the tension and does the tensioner spring back to position when you reduce the force??----if it does---wada you think?

Just spray it with some lubrication and it will ring forever.if these metal ballbearings will drie out then it dies.

I think most tensioners those days are lifetime item (again) but idler pulleys bearings are still weak. You can inspect them easy with belt removed. If they don't spin smoothly- replace. Cheap enough and I think carry spare one in trunk would be good idea.
