I am up for my second B service.
I was quoted in the 400-500$ for the B service.
The person I scheduled with said mercedes recommends transmission fluid change at 40,000 miles and that would bring the total to ~800$.
Is there justification for the transmission fluid change at this time and what they are charging?

I'd recommend getting it changed but $400 to do it seems insane. I'd make them show you the flat rate recommendation from Mercedes for that job and tell you if they change all the fluid and sensor(s) in the transmission.

Does service A, B, C, etc. etc. need to be done at 1 year intervals or are they mileage based? I asked because our car has 6500 miles and the Service Assist says Service in 50 days (which will be 1 year).

Mercedes now goes by 10K intervals or 1 year intervals whichever is less. See the 2010 maintenance manual.
