At 45k an engine mount needed replacement (this requires an alignment afterwards).
58k or so, low pressure fuel line clamp loosened and there was a tiny bit of fuel weep that resulted in a periodic odor. Resolved by refitting it.
Keep an eye on the oil separators for blockage, replace the coolant reservoir every 5 years (silica pack inside breaks down and beads clog heater core), stay on top of specified maintenance (plugs, transmission, diff, 5k oil) and you shouldn't have much to be concerned about. In the S-Class world, there are many high mileage M176s (and 9g transmission) examples with no major fail points to note.
