Normal driving - ML drives great - smooth idle, smooth acceleration - no problems.
Driving fast, then suddenly push the gas pedal to the floor and get the revs up to 5000, then I get the problem.
Problem is that cylinder #6 is dead - i.e. disconnect the spark lead and get no change in idle.
Fixes tried: new plugs, new plug leads, new coil, new air-flow sensor (sensor in the air filter hose between filter housing and manifold), new crankcase sensor.
PLEASE, PLEASE, any ideas.... could it be a faulty/blocked cat converter? or faulty control unit? something else?

You certain its not firing?? Remember there are 2 plugs per cylinder and if one is disconnected you will get no perceptable change. You also failed to state what the mileage is.
