transmission jerk 5th and 6th gear

transmission jerk 5th and 6th gear
Had codes ran but not seeing anything.
When I'm on Highway I'll be in 5th or 6th and I can let go of the gas for a second then reapply and I can feel the gear jerk or clunk.
Searched everywhere and couldn't find any answers.
I used to get issues of getting stuck in first which normally is conductor plate but this is completely different.
Any ideas?
