Power steering lower radiator hose came off!

Power steering lower radiator hose came off!

Hi,
As I was pulling out my driveway yesterday, a very hot day, I noticed a sudden heaviness of the steering. It was the first drive of the day. It just happened as I drove off, it wasn't an overnight thing. I pulled back on the driveway and I noticed that I lost all the fluid on the floor. When I looked under the car I found out that the lower hydraulic cooler hose was completely off the connection. Interestingly, the clamp shown below, was still hanging on the connection nipple of the radiator. The hose had pulled out of the clamp. The clamp was still in the closed position and looked fine. The hose seemed to be fine too. In my nearly 8 years of owning this car, I had never touched this connection. I changed the pump couple of years ago, and that's about it. Putting the hose back with a new clamp is not a big deal. But I'd like to know why this happened.
Any guidance will be much appreciated. The clamp shown below is CLIC E 155.
