Grinding noise

My 2006 e 350 with 76K miles two weeks ago started grinding when idling and the engine is under load such as stopped at a light or idling when the A/C gets turned on. It does not happen consistently but under certain times when the engine comes under load at idle. It sounds like the teeth of a flywheel hitting a shroud, i.e., very fast and it goes away when the tach gets around 900+.
