W212 2012 E Class Power loses often

W212 2012 E Class Power loses often
Hi everyone hope everyone is fine here english is not my first language so apologies for any mistakes.
So i have an E class 2012 w212 the car is pretty old and have done over 300k miles,
and the problem with the car is it has rough idle and hard shifting first to 3rd gear ive done full service gearbox serviced but still the issue same,
But sometimes it becomes very smooth shifting very quickly performance really good but it doesn't last long it vibrates little bit when stationery and after that its still same the old car with rough idle and power loss ive changed engine mounts too,
DPF pressure sensor was changed too,
and there is no codes when i connect it to OBD2.
Can please someone advise what it could be?
