I will say that the R class has significant issues. I would think you would have to evaluate your mileage today; the repair work that has already been done; your personal repair knowledge.
There are lots of threads on the R350 issues.
Basically, the big offenders:
engine balance shaft (I believe there is a link for the affected VIN's)
transmission (conductor plate and control body)
it looks like the front differentials may be showing signs of an upcoming issue.
Rear latch failures
Air-springs
power steering pump/reservoir
intake manifold vanes
That is just the basic big issues. if all of them have been fixed there is less to worry about with regard to those. if you have high mileage, then you have to figure that out in terms of repeat failures .
That are parts known to fail on our R-class are not being covered by this warranty. Also, this is the first time I see an extended warranty "REQUIRED" certain maintenance every year and it means there is a good chance they can deny your warranty claim if you miss some items that can be totally unrelated to your problems. Also, many of the required maintenance items on the list are totally unnecessary for our R-class based on MB service manual and it is a totally waste of your money.
On extended warranty, I would look for one that list the "non-covered" items instead of what parts are being covered. The most important is you need to look for feedback of that company on how "willing" to pay the claims. Your warranty doesn't mean much if there are too much hassle to file the claim or worst that they just keep reject your claims with some funky reasons or legal holes.
Good luck.
