W220 juddering at low revs

I have a 2003 s320cdi that has a tendancy to judder when under load at low revs (worst on a gentle incline at 1200rpm / 40mph). If you accelerate or slow down then noise disappears.

From reading various posts I believe this may be a problem with the torque converter. If so is it worth trying a gearbox fluid and filter change? atf spec, volume?

Also how difficult is it to replace the torque converter? Any special tools required?

On reflection it is more of a rumbling sound than a judder, as though you were driving over a rumble strip.

I am going to change the gearbox fluid and filter to see if this helps.

Replaced the motor and transmission mounts.

The old ones were slightly compressed by about 1cm however this did not solve the juddering problem.
