Hello, just started a search for a lightly used S550 (less than 40k miles) and would welcome insight/advice.
Is there a better year between the model years 2007 vs. 2008 vs. 2009 vs. 2010? What is typical maintenance cost and issues if any? As foolish as it sounds, is avg mpg really 16? Driving an '89 560sel and getting 13mpg is a bummer.

To start, MPG is closer to 17-18 in the city and upwards of 27 on the highway. In regards to year, you'll probably get some preference from other owners, however I would recommend staying away from early model 2007 S550's due to transmission and engine issues fixed in later models (for the most part).
I think in terms of the years you listed, I would get the newest model year with the lowest mileage that your budget allows. IF POSSIBLE, I would definitely go for a 2010 facelift model, which will give you all of the newer LED aesthetics.
In terms of maintenance, like any import, if you maintain it and take care of it, there's a good chance it will take care of you. That said, keep in mind the S Class is a $100k car and maintenance will reflect that. The most important thing is to find yourself a CPO warrantied vehicle, or find a good independent you can build a relationship with and seriously cut down on your maintenance/repair costs. As for common issues/repairs with the different model years, I would recommend reading through the W221 forums here, there are many threads about the common issues with lots of great information.

MPG is much better than your 560SEL. These vehicles can easily get 24 or more MPG at freeway speeds in steady traffic, sometimes as high as 27 MPG. In addition to better mileage, there is a lot more power reducing the time from 0 to 60 to about 5.4 seconds.
Everything in this vehicle is more advanced and complex which can lead to very expensive repairs if something breaks. Most vehicles have had good reliability. Routine dealer annual maintenance costs in Southern California is between $160 and $300. Tires can easily cost $400 each. Transmission needs to be serviced one time at about 40,000 miles (under $500). Spark plug replacement is a major cost (2 plugs per cylinder).

Recently did a long trip and got 25.9 mpg on the highway going 74mph with the air on! These cars are underrated from what the window sticker says for highway mileage!!!
