No MMI port on W221
either way the strange proprietary plug you are referring to allowing various cables to give a more interesting set of Aux in connections came with a later facelift.... they got a newer less reliable version of the comand system moving from the 2005 NTG3.0, with the later (2010 model year) cars coming out late 2009 with a NTG3.5 unit (which is really an early NTG4.0 as used on E classes)

it was only to allow a second option on LWB cars that get a bluetooth handset in the back to talk to a clipped in phone up front - thus your bodyguard can't hear you making the call to kill his mates working for the other company
very low spec cars don't have the dial pad behind the comand knob - if this opens to show a phone pad you should have a UHI socket in the center console chubby hole - this allows Merc or aftermarket toys to clip in and turn the UHI into a BT phone connection.... BUT the clipped in phone had a good signal (with external antenna) - if you have the funny pink tinged IR reflecting glass option, your signal strength drops 2 to 3 bars
these days we can get music streaming and BT calls and if stupid use spy mapping via modern add-ons like
