When topping off the coolant reservoir should it always be the 50/50 mix?

1. Can you top off with just water?
2. Can you top off with just coolant?
3. At what level is it no longer just topping off and becomes more of a refill?
4. What are consequences of using just coolant to top off?
5. What are consequences of too high a ratio of coolant to water?

i use straight coolant

i dont think coolant evaporates, im pretty sure its just the water in the mix that does so i just top off with distilled water
