Only Shardul and Brooke aka UPD sells them to my knowledge. They use to be $70 back in 2014. He recently quoted me $170 for one.

Fortunately I already have a modular crank, and pin kit (got a killer deal on a used set up). Just need a new bolt. Turns out the OEM bolt will not work with my kit (UPD) and probably most kits because of the raised edge around the base of the threads. With the crank extension/insert a bolt with a perfect 90 from the bolt head to threads is needed. If I cant get a bolt for a reasonable price my plan is to machine a beveled edge on the insert to allow the factory bolt to seat correctly.Originally Posted by VictoryRoadPerformance
