I preemptively replaced one voltage regulator at 150,000 miles and on the other E55 I replaced the voltage regulator at 160,000 miles. In each case I estimated they would have been failing by 170,000 miles based upon the wear of the brushes compared to new. So, based upon my admittedly small sample size of two data points, I would recommend replacing them at 150,000 to 160,000 miles.

For the E63, anyone know if the voltage regulator be serviced from under the car or does the entire alternator need to come out to access the voltage regulator on the back? I'm trying to find out the easiest way to swap it.
