W203 C280 2006 4Matic Multiple symptoms

Hello everyone!
I am new to the community and was looking for some help.
I have a W203 C280 2006 4Matic Benz and I am running into some heavy problems. For that reason I bought a simple scanner and will provide as much info as I can. These symptoms only show after car warms up.

The First Problem:
My Car idles rough at 570 rpm and does many rpm surges to 600 and drops again and then again goes to 600.
What I have seen with the scanner
  • Ignition timing advance for the 1st cylinder goes negative -1, -2, -3 BTDC and then +5
  • MAF air g/s goes to 0 and car acts like it will stall and still stays under 3 g/s even though i changed it and cleaned it
  • Short Fuel Trim stay and move around 25% when the scanner says max it should stay is 15%
  • Short Fuel Trim B1S2 and B2S2 are both at 99.2%

The Second Problem:
When Vehicle has been drove for more than 20 minutes and is warmed up when you turn the wheel all the way the car jerks forward and backwards and the tires look like they not moving in synch. Can it be the transfer case?

The Third Problem:
After car has warmed up when you drive at 1100 rpm it makes a big clunk noise at the back. Another symptom is when you come at a full stop the car jerks and pushes you forward while making the same noise. Is it related to the 2nd problem?

The Fourth Problem:
After driving for more than 30 minutes switching from P to D or R gets harder and makes clunk noise and pushes car forward. Is my transmission gone [img]data:image/gif;base64,R0lGODlhAQABAIAAAAAAAP///yH5BAEAAAAALAAAAAABAAEAAAIBRAA7[/img] ?

The Fifth Problem:
After car has warmed up releasing the foot from the gas pedal makes the car drop rpm and it feels like it is braking while immediately lowering the speed. Also it happens pretty commonly that as soon as it changes gear the rpm drop immediately and car jerks.

I know that I have a lot of problems and that I should just send and leave my car at a service shop, but I wanted to get some information from the community first if it happened to anyone else. Every little help is appreciated. Should I change transmission and Throttle body even though they are not showing any Engine code? Oh, btw my Engine light is on and it is showing error code p2421 code related to EVAP system.
