Expansion tank cap

Expansion tank cap
My car needs an antifreeze top up every two weeks or so, the tank is empty.
There's no sign of a leak but it could be anywhere.
My feeling is that the expansion tank cap is loose so steam is escaping. Is that a thing? It feels too easy to turn when the engine is cold and there's no rubber seal inside the cap. Should there be?
It's a 2012 2.1 diesel engine.
Any ideas?
