Damaged Break Rotor - No Warning Light

Damaged Disk - no warning light.
Is it possible to have the break pads worn so unevenly that the break rotor is damaged - there was no warning light or anything? The car was in MB dealership for service a week ago and yesterday, when I heard the grinding noise, I saw the damaged left rear disk. I think the sensors are only 2 (front/rear) passenger side.
