I have had SD warranties on a E350 4 Matic, an SLK350, a CLA45 AMG and this one all bought, transferred, refunded and rebought through my dealer.
I have had one repair under the XTD warranty to replace the AC evaporator which cost about half the premium. One more of that magnitude and it starts to pay for itself.
If you buy junk bonds be prepared to lose. Same on a warranty.
There is a 2015 507 which is the last of the W204s with 14000km still under warranty in Calgary selling as a MB Certified Pre Owned so you get the MB extended warranty and then you can tack on something like an SD without inspection. Lots of options

Since the A/C evaporator was mentioned, in humid and warm areas there is a bad odor from the A/C that is unfortunately common for many MB vehicles not only W204. In your climate you should be OK. Replacing the evaporator is a big PITA, the dashboard needs to come out.
Now for the costs or reliability, the W204 is not bad even out of warranty. I've owned over the years e36/e46/e90 BMWs, Audi A4, 996 Porsche, Lexus and others to compare. The C63 is very good so far.
Several reason for it, the W204 is before the era of electronically adjustable everything such as suspension, engine, steering. It's naturally aspirated (non-turbo/supercharged) and equally as important the M156 engine is not directed injected.
