coolant leak

Hey guys , yesterday I was driving and I got a low coolant light . By the time I got to an exit the car started overheating . When I opened the hood I can see water spewing out from where the upper radiator hose connects to the radiator . Before I go and start throwing parts at it . Is this just a loose hose or do i need a new hose and or radiator ? The reason I ask is because i know the hose uses a spring clamp . But I am not sure if they can come loose . I appreciate any help .
