Headrest adjustability ?

Why don't car manufacturers design headrests that can be adjusted forward or back as well as up or down. The current configuration requires me to lean my head backwards into an uncomfortable/unsafe position. The bmw 7 has the ability to tilt the upper part of the seat forward and the headrest has "wings" which pull out so it comes close to a usable headrest. I almost wonder if there are liability concerns about someone getting too comfortable with a usable head rest and falling asleep.

The E Class headrest is hinged at the top. Pull it forward from the bottom and it will come closer to your head.

Wife, headrest

And my wife eants it BACK - she's not tall so the headrest pushes her head forward (and yes it is in fully back position). ANyone with the same issue? How did you deal with it?
