It's actually a bit bigger deal than that. Cars with the top fuel cap are essentially FlexFuel vehicles (FFV) while the ones with the bottom are not. Some cars come with build option code 929 - ENGINE FOR ETHANOL FUEL while others don't. I was wondering what was the differentiating factor but so far I have found none.

Based on the small sample who responded most people don't have a FlexFuel ready E350 and they don't even know it was an option, myself included. Had I not gotten a loaner with the weird fuel cap I would not have known either.
I'd also note that with the FlexFuel E350 I could see the ethanol percentage as an ODB2 value and it was around 6%. That value bank is not available to read with the regular E350.
