Weird idle issue

My 10 E350 has developed a rough idle after it warms up and is now causing the CEL to some on. The rough idle is intermittent and doesn't always cause the CEL to come on; I cleared it to see if it would pop up again and it did. I used my OBD II reader to see what code it is and it's coming up with both banks running too rich. I recently changed spark plugs to the NGK Platinums. The car runs just fine when driving both city/highway and I'm getting better gas mileage with the new plugs, but I'm really concerned about the intermittent rough idle. If anyone has had this problem or better yet has a solution I'm all ears. Thanks.

Have you tried cleaning the MAF sensor? When you put the air cleaner housing back on did you connect the rubber tube at front? Stock air cleaner elements?

Have the code read by STAR diagnostic then there is no guess work.
