It's a plug-and-play installation but it does require tearing apart your dash to remove the radio head unit, accessing the wiring harnesses, and then putting it all back together.
It would also be a trivial installation for an experienced installer with Mercedes experience.

For example, some have put the unit above the passenger footwell cover or under the carpet at the top of the passenger footwell. In my case, I simply placed it on the upper shelf of the glovebox so I'd have easy access. (And I don't really need to use that part of the box for anything else.)
FYI, I did a DIY for the NTG45./4.7 unit on the W212. I'm assuming the 4.0 install is similar and the DIY has links to instructions and videos for the W212 interior disassembly.

Curious with Couple Questions!
Hey guys, does an after market car play cause electrical issues down the road? If none, how do you handle the software upgrades? And Would you recommend getting the CarPlay after you’ve installed?

This is ONLY for Apple car play right? I noticed on their site they have some models for the s class that do both android auto and apple car play. But the one for the w212 only does Apple Car play? I just want to make sure I am understanding.

Essentially I found a joyeauto CarPlay wireless module and booked an install with an audio specialist. I was just curious who’s done it on their car before me..

FWIW, I have two MB autos (Comand NTG4.7) with aftermarket boxes installed, one Joyeauto and one Road Top. Both boxes support CarPlay and Android Auto although I have an iPhone so I have not played with AA.

You can find vendors on fleabay, amazon, alibaba, aliexpress, etc.
2015 E400 COMAND 4.7 with wireless CarPlay using Joyeauto smartphone module. The wifi is fast and stable, all OEM functions retained, though I now use CarPlay apps for music & news. Since I'm using the stock monitor, I have to use the scroll wheel to navigate, but that's not a big deal for me...
