2011 E350 jerks from D to R and vice versa

2011 E350 jerks from D to R and vice versa
Does anyone have had this problem?
When I put the gear from D to R the car kicks forward like someone is hitting it from behind, same thing when going from R to D. This doesn't happen when the car is cold in the morning.
Also when I accelerate aggressively the RPM's go up (not sure but from which gear to which gear) and stay around 4000 for a second or two and then on gear change there is the kick again like someone is hitting the car from behind. Same thing happens when I come to a stop (not every time though).

Have you checked the motor mounts/transmission mounts? Very common for those to go. The transmission mount is easy, the engine mounts not so much.
