Damage History on VMI: Red Flag?

Hi all,

Is there anything in this VMI to be concerned about (first attachment)? Specifically, anything in the damage history section? Or any other red flags for that matter?

Also, not all boxes on the CPO checklist are checked off. Any issues there (second attachment)?
