Loud Metallic Clicking Sound
Starting from a dead stop or slowing down in low speeds I hear a metallic clicking sound which I think is coming from the rear. Suspension or brakes? My mechanic says no, he thinks it is transmission related. Started 6 months ago very soft sound only on acceleration from stop but has progressed to louder and more frequent than including slowing down. Really sounds like suspension to me. 180,000 with all required service performed.

Could be torsion bar bushings or transfer case. If you're sure it's coming from rear, and not front end (cv joints), when was the last time you changed your transfer case fluid?
If your mechanic is saying likely transmission, what evidence is he pointing to?

Stuartames, Welcome.
Please update your user CP info.
ml320 just doesn't cut it.
Could be transfer case as suggested by we47b1me (as they upgrades some of these back in 98).
SO until members here specifically know what you specifically drive, guessing is a waste of time.

If your mechanic doesn't know, time to get a new mechanic.

It is hard to tell if the noise is only coming from the rear. Today it was less metallic and more squeaky like bushings. Transfer case sounds serious.
My mechanic claims to have throughly inspect the suspension and brakes but couldn’t find anything wrong. I trust him but we all make mistakes. I probably should take it somewhere else for a 2nd opinion.
I understand these early US models had transmission problems.

1999 ML320, extensive service less than 1 year ago including transmission fluid, filter, differential, etc.

Bulletproof. Yes, bulletproof.
Why is he looking at the brakes and suspension??
Get a new mechanic. He sounds like an idiot. If he doesn't know these cars, he shouldn't be touching yours.
Ask him to inspect what I suggested.
You also did not answer my question.

Verify your mechanic checked the torsion bar bushings. Once they fail, it's metal-on-metal grinding.
Also investigate this as you are still not sure if it's noise from the front end or rear end.
