I'm looking to buy the MY06-08 ML350 w/ about 50k-60kmi. Price is very good now, ~$22k. Besides the infamous transmission rough shifting issue, which was probably due to lack of transmission fluid change at the 40k intervals, are there any major problems, such as fuel pump, common/nav system module, ball joints, cooling system, etc?

I found the Nav system is so outdated in the 06-08 models. If you can wait get a 09+ for led tail lights, minor body change, etc.

Mine is 2006 ML500 60,000 mi. Recently the power steering fluid was leaking and had to change the pump which is common issue for early W164 model. I went to indy shop but since new Mercedes part was almost $1000, I tried a rebult one but with labours + taxes, it still cost me close to $1000.
Now there is annoying squeak noise from rear suspension, turned out to be the rear bushings need to be replaced for $350.
If you're looking for early models, you should spare some cash for repair bills.
