What is your definition of worth ? Are you going to recoup that 10K back during trade-in or resale time ? Not a chance. The leather and touch is real nice in designo - but its basically if you like it. It certainly is different than the 3 interior choices that MB gives you (although exterior choices are close).
How about this - if you're asking whether its worth it, its probably not for you. It's absolutely not worth it but people who get it(or got it) don't/didn't really care whether it was worth it or not.

If you're looking to buy a 220 with designo (used obviously), then of course the premium would be much less than $10k. So if I was looking at a 220, I'd be looking at designo cars if possible.
If you're buying a new car, it all comes down to how much $10k is worth to you and what other options you want on the car. In my case, the other options I could get already put the MRSP over 110k. Adding designo would have been getting too close to the S63 or S600 in my opinion. I wasn't comfortable paying so much for "just" a 550. But it's a judgement call. Some would argue even $110k is too much for just a 550.
Also, it's really closer to $8k on the 221, when you consider a lot of people pay for the premium leather anyway.

I would point out two other things. I've owned Porsches with and without the full leather doors and dash and over time you really appreciate the full leather panels. Which is definite plus for the designo cars.
Two, on my 221 the material on the A pillar and headliner is absolutely identical to the material in our Touareg. Not as nice as the material on our 201 or 124. And not nearly as nice as the alcantara I've gotten used to with Porsche.

My car is a designo silver and I love the color. It actually looks faintly green instead of silver. As for the worth of the designo package....I wouldn't have paid extra for it in 2003 but as a used car I don't think it added a whole lot to the value. The seats also have an embossed "designo" on them. Not sure if the quality of leather is actually any better but it looks very good.
