It has a very high probability of happening. My upper right pulley went bad after 18000 miles and it seems to happen only on S600s as far as I know. When they replaced the pulley, they also replaced a connecting part also that had to be modified. Apparently this is a known issue as konigster has pointed out and they are trying to repair it with a better designed alternative pulley assembly.
It's about a 2 day job assuming they have the parts on hand.

12,000 miles in and no rattle yet. I will look out for it though... I have not been putting on the miles as much recently as I often commute to work (and exercise) by bike..

The contrasts are interesting.. one day at the bottom of the traffic hierarchy on my bike.. the next dat at the top in my S600...

This might be related. When I had my 2007 CL550 in for service last month, the dealer replaced the idle pulley because of a service campaign relating to noisy pulleys. Mine was not making any noise, but they did it as a preventative measure. I don't know if the same pully is used for V12s.

Recently I made a comment on another thread that I thought my pulley was going bad again. I don't think it is as I notice the whining sound only on very cold mornings. Once the car warms up or the temps rise throughout the day, etc, the moaning, whiny sounds goes away. It is really bad between 1000 and 2000 rpms. My thoughts are that the serpentine belt is stiff and tighter and just makes more noise at these low temps (usually below 35).
After a good warm up and a few miles down the road, the car is quiet as can be. Probably the reason I notice it so much is because I don't have far to drive to work (1.3 miles) and so I notice it more often.
