I completely get that the lug nuts should be tightened according to manufacturing specifications and procedure. Assuming that, I suspect that a wheel fly off occurs about as often as propeller driven aircraft sheds a propeller blade. Both catostophic events. Neither of which happen frequently enough that you ever hear about it. Kind of like fatalities, or injuries for that matter, from elevator failures.

By the way I in know way want to distract from the OP's interstate disaster. Whatever the reason, that was awful and the cause should be discovered. Unless you have the failed parts under analysis, all of this is pure speculation.

I had just bought the car and this was my first road trip with this car so I don't know if the dealer worked on the wheels or not but either way they ordered a new wheel, tire and lugs at no cost to me.
