Screw in tire
2018 s450 with 8k miles and the tire light came on with the tire pressure at 23 lbs. screw ended up being in the tread about 1.5 inches from the inside of the tire. My local mechanic put a plug in it and said where it is I would be totally good. The tires are the factory Pirelli 19 inch run flats. What do you guys think, plug good? Pirelli’s warranty wouldn’t cover that right? Should tire be replaced?

did you purchase the tire/rim coverage ? if yes get a new tire, if not sounds like the patch is fine. plug might be ok... Ive certainly used them... but a patch inside is better....

Just Had Tire Replaced Under Warranty
I have new Pirelli run flats with less than 2,000 miles on them on my 2015 S550. I picked up a nail in the left front. MB replaced the tire under the wheel and tire warranty. $50 deductible.

If the plug stops the leak you are good. If not, then an inside patch is 2nd option and best remedy. If involving the sidewall, get a new tire. I plug my own tires by using a drill, then an Autozone plug kit. Works 95% of the time. I had to have my right rear tire patched though last month due to the fact the tire shop refuses to plug tires. No big deal.

Well it sounds like you got really screwed. But maybe not. I believe Pirelli has a tire warranty on their run flats that covers run flats and they say that their tire should not be repaired. It's under their special warranty information. Looks like it should be free except for mounting/balancing as I assume you've had it for less than a year.

Road Hazard Policy for Run Flat Tires Only
Pirelli Run Flat tires are manufactured with technology that allows limited low-inflation operation in the event of sudden loss of pressure. Pirelli does not recommend the repair of run flat tires and as a result, Pirelli provides road hazard coverage on all Pirelli produced tires that are manufactured with run flat technology. To qualify for Run Flat Road Hazard coverage, a completed claim form along with the tire exhibiting the road hazard must be returned to Pirelli through an authorized Pirelli dealer.
If a Run Flat tire becomes unserviceable due to workmanship or materials anomalies or road hazard injury during the initial warranty period, which is one year from the date of original retail purchase of the vehicle or purchase of replacement tires (purchase receipt required) or within the first 2/32” of the original usable tread, whichever occurs first, the tire will be replaced with the same or comparable tire at no charge for the tire to the owner. After the initial warranty period, if a Run Flat tire becomes unserviceable due to workmanship or materials anomalies or road hazard injury, the owner must pay the cost of a comparable new Pirelli replacement tire on a pro-rata basis. The authorized Pirelli dealer will determine the cost by multiplying the percentage of the original usable tread worn by the current dealer selling price. The owner must pay for any associated service charges, including costs associated with mounting and balancing of the tire. This Road Hazard Policy is not a warranty. Pirelli does not warranty that a Pirelli tire will not fail due to road hazard and does not authorize any person, including authorized Pirelli dealers, to make such a warranty.

Good call on the road hazard warranty, talked to Pirelli and they said they would warranty it. They also said sometimes the dealer won’t want to do it unless you bought the rim and tire package.

Run flats are typically not to be repaired per manufacturer. Many tire shops will also refuse to do so. Having grown up in Germany and driving at high speeds I learned that tires and brakes are the most important parts of your car. Perhaps a bit less critical in the US, I will always replace the tire. A few bucks isn't worth risking anyone's health...
